Position Purpose: To provide outreach, education and connections to community resources; assisting people to navigate systems to reduce or eliminate barriers to pursuing recovery

Principal Responsibilities:

  • Conduct community outreach about substance use disorder and treatment/support options. Understand current public health and public safety needs within the community.
  • Network with and recruit community-based organizations, faith-based organizations, and other community groups to become a part of an active locally based support network for people challenged by substance misuse.

  • Engage individuals at the referral location, establish rapport with the individual, and assess needs to determine what services and support can be provided.
  • Identify client’s wishes and develop an individualized strength-based plan, including navigating systems and connecting with resource to achieve their recovery goals, including exploring the full range of treatment options available to them.

  • Connect individuals to all available resources and coordinate which existing services the individual is receiving to prevent duplicative efforts.
  • Maintain contact with the client and/or other individuals or agencies for the purpose of meeting the goals and objectives defined in the case management plan. Contact may be face to face or via phone.
  • Provide coordination and communication with individual’s support system, as needed.

  • Maintain daily documentation and update data for both weekly and monthly reports.

Preparation, Experience and Skills:

  • Recovery Coach with CCAR/CPRC/CADC and 3-5 years of experience in SUD field OR
  • Associate degree in social work, psychology or related human service field with 1-3 years of relevant experience (preferred).
  • Secure Community Health Worker (CHW) credential within first year of employment
  • Strong preference for a person with three or more years of active, continuous recovery.
  • Demonstrated competency in alcohol, drugs, substance use disorders and recovery, including a working knowledge of the SUD continuum of care and insurance.
  • Demonstrate compassion, empathy, respect, flexibility, patience, persistence, and hope with all individuals served regardless of their level of need or stage of recovery.
  • Adhere to established professional codes of ethics and standards of practice.

  • Demonstrate an understanding of the need to identify, evaluate, and manage boundary issues effectively with an emphasis on modeling healthy boundaries.
  • Excellent organizational skills, comfortable with technology (electronic medical record, Office365, etc)
