Job Description
We are seeking a Home Based Services QIDP to join the Social Services team. This position is responsible for the development of comprehensive services that support individuals to achieve their personal outcomes, coordination of services, overseeing the implementation of Service Agreements, and providing the general case management to ensure the individuals with intellectual and physical disabilities in services attain personal goals.
Schedule: This position is Monday - Friday 8:00 AM - 4:30 PM. The salary for the position is $44,000 - $52,000 (depending on certification/experience level).
Responsibilities: - Develops, monitors and documents Service Agreements, Chronological Logs and Implementation Strategies (IS)
- Explains all rights to which an individual is entitled as required by DHS
- Reviews service agreements monthly, documents service implementation, progress, and needs for modification
- Provides a copy of a written service agreements to the individual and an electronic copy to each member of the individual's community support team
- Ensures all areas of responsibility are in compliance with CARF, DMH/DD, etc., standards and/or regulations
- Advocates for services and supports to help the consumer achieve their outcomes
- Assures consumer's participation in all decisions affecting his/her life
- Serves as primary contact person for guardians and all service providers
- Works with Independent Service Coordinator (ISC) to facilitate Personal Plan, discovery and CST meetings
- Ensures that all additional assessments are completed in a timely manner
- Develops a Service Agreement that incorporates all needed services for the consumer
- Evaluates the services and implementation of treatment plans to ensure appropriateness for the individual and the compliance with regulatory requirements
- Trains families, guardians and personal service workers on implementation of the service agreements
- Ensures service agreements and implementation strategies are completed for tracking individual outcomes
- Completes written monthly review of the consumer's Chronological log and submits complete hours of service tally to director in a timely manner
- Assures that all services are delivered as outlined in the service agreement
- Completes home visits in person or virtual as required
- Attends the CST for discharge planning when invited by individual
- Completes documentation reviews as required by Quality Assurance
Qualifications: - Must be 21 years of age
- Must be a DHS certified QIDP
- Bachelor's Degree in Human Services field, including, but not limited to, sociology, special education, rehabilitation counseling or psychology
- Two years of Self Directed Assistance experience required
- Pass a criminal background check and physical/drug test
- Valid driver's license
