Job Description

The Social Worker Care Manager is responsible for providing appropriate interventions and discharge planning services to patients and families and facilitates a smooth transition for the patient throughout the continuum of care by accessing hospital, community, and governmental resources. They also provide clinical supervision to peers, Social Workers, and students. Essential Job Functions include identifying and prioritizing patients in need of social services using a holistic approach inclusive of biopsychosocial, functional, cultural, spiritual, and financial factors; planning with the patient, caregivers and healthcare team to maximize healthcare responses, quality and cost-effective outcomes; monitoring and revising plans as patient condition changes; completing all necessary documentation reflecting plan of care and patient involvement; following standardized practices related to Advance Care Planning, Length of Stay management and readmission prevention; supporting denial prevention related to medical necessity; promoting assertive, proactive care and removing barriers to timely testing and treatment; collaborating with revenue cycle partners and patient access to ensure appropriate classification and regulatory compliance; participating in clinical outcome projects and process improvement initiatives; working collaboratively with peers and care team members; supporting compliance with CMS and Conditions of Participation; staying current on community resources and clinical advancements; providing supervision for social workers and students. Additional duties may be assigned by supervisors. Licensing/Certification required includes BLS Basic Life Support and Licensed as a Social Worker in state of practice (preferred in VA). Accredited Case Manager Certification or Certified Case Manager preferred. Education required is Bachelor of Social Work, with Master’s degree preferred. Experience required is 1 year in clinical setting, with 3 years in acute care preferred, and ambulatory or post-acute care coordination experience preferred.
