Are you passionate about patient care and creating a positive first experience for those navigating chronic health conditions? We’re looking for a Patient Enrollment Specialist (PES) to join our growing team and play a key role in supporting the onboarding of patients into our Chronic Care Management (CCM) program.
About the Role
As a PES, you will serve as the first point of contact for new patients. You’ll be responsible for educating patients on CCM services, obtaining consent, creating the first care plan, and ensuring a smooth transition to the long-term care team. What You’ll Do
•Conduct initial outreach calls to introduce the CCM program and obtain patient consent.
•Develop and document the first care plan in alignment with CCM requirements.
•Complete the initial CCM report before transitioning patients to their long-term Care Team Coordinator.
•Ensure all documentation is accurate, timely, and compliant with regulatory standards.
•Work closely with internal teams to support seamless patient transitions.
•Address any questions or concerns patients may have during the onboarding process.
•Support various administrative tasks related to enrollment and care coordination.

•Ability to work independently while contributing to a collaborative team.
•Experience in healthcare, care coordination, or customer service preferred.

Compensation:
$19.00 - $21.00/hour + Performance-Based Bonuses
