About the Role:

The MDS Coordinator plays a critical role in ensuring that the Minimum Data Set (MDS) assessments are completed accurately and timely, which is essential for compliance with federal and state regulations in the healthcare sector. This position is responsible for coordinating the MDS process, including the collection, analysis, and reporting of patient data to optimize care planning and reimbursement. The MDS Coordinator will work closely with interdisciplinary teams to ensure that all assessments reflect the true clinical status of residents. By maintaining high standards of documentation and communication, the MDS Coordinator contributes to the overall quality of care provided to patients. Ultimately, this role is pivotal in enhancing the operational efficiency of healthcare services and improving patient outcomes.

Minimum Qualifications:

  • Bachelor's degree in Nursing, Health Administration, or a related field.
  • Current RN or LPN license in the state of practice.
  • At least two years of experience in a healthcare setting, preferably in long-term care.
  • Certification as a MDS Coordinator (e.g., RN-BC, CDP).

Preferred Qualifications:

  • Experience with electronic health record (EHR) systems and MDS software. PCC specifically.

Responsibilities:

  • Coordinate and oversee the MDS assessment process, ensuring compliance with regulatory requirements.
  • Collaborate with nursing staff, therapists, and other healthcare professionals to gather necessary patient information.
  • Review and analyze MDS data to identify trends and areas for improvement in patient care.
  • Provide training and support to staff on MDS-related policies and procedures.
  • Prepare and submit MDS assessments and reports to appropriate regulatory bodies in a timely manner.
