Job Description

Transplant Coordinator RN at Providence Sacred Heart Medical Center Hospital, Spokane, WA

Provides coordination/case management for the transplant related activities of patient caseload across the continuum of care – pre-transplant, through post-transplant. Provides patient/family education, coordinates patient evaluation, maintains waiting list registration and maintains patient readiness for deceased donor transplants. Arranges transplant admission, monitors clinical pathway progression and provides post discharge follow-up. The coordinator provides professional/peer education, and participates in the development of patient education programs.

Required Qualifications:

  • Bachelor's Degree in Nursing

  • Upon hire: Washington Registered Nurse License

  • Upon hire: Remains up to date on UNOS-OPTN and CMS regulations

  • 5 years Acute care nursing experience

Preferred Qualifications:

  • Upon hire: Certified Clinic Transplant Coordinator (CCTC) certification (maintains)

  • 3 years Current experience in Cardiology nursing, including transplantation
